Abstract:
Access to quality healthcare services remains a critical challenge across India, particularly in semi-urban and rural regions where doctor-to-patient ratios are severely skewed and appointment management is manual and inefficient. This paper presents MediNexus AI, a full-stack intelligent healthcare appointment and consultation platform that integrates a multilingual AI medical chatbot, real-time WebRTC-based video consultation, OTP-secured appointment booking, and a Razorpay-powered payment gateway into a unified, accessible web application. The system adopts a three-tier architecture with a React.js Single-Page Application (SPA) frontend, a Node.js/Express.js REST API backend, and a MongoDB document store, augmented by a Socket.IO real-time layer for peer-to-peer video calls.
The AI chatbot component leverages Google Gemini (gemini-2.5-flash) with a six-model fallback chain to provide symptom triage, specialty recommendation, OTC medicine guidance, and emergency detection in four languages: English, Hindi, Marathi, and Hinglish. Key security provisions include three-role JWT authentication, HMAC-verified payments, rate limiting, and Socket.IO room access validation. A comprehensive doctor management portal enables appointment oversight, earnings dashboards, prescription delivery via email, and medical record uploads through Cloudinary. Evaluation demonstrates complete feature coverage across appointment lifecycle management, real-time communication, AI-assisted triage, and administrative control. MediNexus AI bridges the digital healthcare gap by making specialist consultation accessible, affordable, and immediate, with future roadmaps targeting native mobile deployment, federated ML-based diagnosis, and EHR integration.
