Abstract
Job satisfaction among nurses is a critical determinant of health-system performance, shaping care quality, patient safety, and workforce stability. In Telangana, as in much of India, public and private health facilities face a persistent nursing shortage compounded by inadequate staffing ratios, limited career progression, psychological strain, and rising outmigration of skilled nurses. This paper presents a detailed, literature-based synthesis of the health-system-level determinants of nurses' job satisfaction and examines their implications for workforce retention and policy reform within the Telangana context. Using Herzberg's Two-Factor Theory as an organising framework, the review distinguishes hygiene factors — staffing adequacy, compensation, physical work environment, and job security — from motivator factors — recognition, autonomy, supervisory support, and career advancement. Drawing on empirical studies conducted in Telangana, comparable Indian states, and international comparators, together with national policy documents and WHO guidance on rural health workforce retention, the paper argues that dissatisfaction in Telangana's nursing workforce is driven primarily by unmet hygiene needs — chronic understaffing, compensation gaps, and burnout — while sustained retention depends on strengthening motivator factors through structured career pathways, participatory governance, and psychosocial support. The paper further considers the rural-urban disparity in nurse deployment within the state and its bearing on equitable access to care. It concludes with a detailed set of policy recommendations for state-level workforce planning, positioning nurse job satisfaction as a foundational pillar of Telangana's universal health coverage agenda.
