Abstract - The increasing demand for accessible, intelligent, and patient-centric healthcare services has accelerated the adoption of telemedicine and Electronic Health Record (EHR) systems. However, many existing healthcare platforms operate in isolation and lack predictive capabilities for proactive healthcare management. This paper presents AI-Enhanced MediCard, an integrated smart healthcare platform that combines telemedicine services, secure EHR management, appointment scheduling, prescription management, and machine learning-based disease prediction within a unified framework. The proposed system utilizes patient demographic information, clinical parameters, medical history, and symptom data to predict disease risks using machine learning algorithms. A Random Forest-based prediction model is employed to improve diagnostic support and enable early intervention. The architecture incorporates role-based access control, secure data storage, and cloud-enabled healthcare services to ensure scalability and privacy. Experimental evaluation demonstrates improved healthcare accessibility, efficient record management, and disease prediction accuracy compared with conventional healthcare systems. The proposed platform contributes to the advancement of intelligent healthcare ecosystems and supports the transition toward preventive and data-driven medical care.
