Abstract - The fast digital changes in healthcare have led to a big increase in the use of telemedicine systems. These systems allow for remote consultations and make it easier for people to access medical services. However, one big challenge is the burden of manual clinical documentation. This process takes up a lot of time for healthcare professionals. Doctors must record patient interactions, write prescriptions, and create summaries. This often results in a heavier workload and less efficiency in patient care. This paper introduces MediScribe AI, a telemedicine platform designed to automate clinical documentation using speech recognition and natural language processing (NLP) techniques. The system captures real-time doctor-patient conversations, turns speech into text, and processes it with AI to pull out key clinical information like symptoms, diagnoses, medications, and treatment recommendations. Based on this data, the system automatically creates organized outputs such as doctor notes, prescriptions, patient summaries, and lifestyle advice. The system uses a modern full-stack architecture. It incorporates Next.js and React for the frontend, Node.js for backend processing, and MongoDB for data storage. Tests show that the system greatly reduces documentation time, improves the accuracy of medical records, and enhances communication between healthcare providers and patients. This approach offers a scalable and effective solution for today’s telemedicine needs.
